I, the undersigned parent or legal guardian of the participant named in this registration ("Player"), hereby authorize FBI Academy and its coaches, staff, and designated representatives to obtain emergency medical care for my child in the event of injury or illness occurring during any FBI Academy practice, training session, clinic, tryout, game, tournament, or other Academy-sponsored activity, in circumstances where I cannot be reached immediately.

This authorization includes permission for a licensed physician, emergency medical technician, athletic trainer, or hospital emergency staff to examine, treat, and, if a qualified medical professional determines it necessary, hospitalize, administer anesthesia, order diagnostic tests, or perform surgery on my child.

I understand FBI Academy will make reasonable efforts to reach me or the emergency contact(s) listed on this registration before treatment is given, except where immediate treatment is required to prevent further harm.

I certify that the medical information provided in this registration (including allergies, medications, and existing medical conditions) is accurate and complete to the best of my knowledge, and I agree to promptly update FBI Academy of any changes.

I understand that I am responsible for all costs of any medical treatment provided to my child, and that FBI Academy does not carry medical insurance covering such treatment on my behalf. This authorization remains in effect for the duration of my child's participation in FBI Academy programs and until revoked by me in writing.

In consideration of my child being permitted to participate in FBI Academy programs — including practices, training sessions, clinics, tryouts, games, tournaments, and related travel (the "Activities") — I, the undersigned parent or legal guardian, agree to the following on behalf of myself and my child:

  1. Assumption of Risk. Youth soccer and related training involve inherent risks of injury, including sprains, fractures, collisions with other players, concussions, and, in rare cases, serious or catastrophic injury. I voluntarily accept these risks on behalf of my child.
  2. Release of Liability. To the fullest extent permitted by law, I release, waive, and covenant not to sue FBI Academy and its owners, coaches, staff, volunteers, board members, and the owners/operators of any facility used for Activities (the "Released Parties") for any claim of injury, loss, or damage — including death — arising from my child's participation in Activities, except to the extent caused by the Released Parties' gross negligence or intentional misconduct.
  3. Indemnification. I agree to indemnify and hold harmless the Released Parties from any loss, liability, or cost, including attorneys' fees, arising from my child's participation in Activities.
  4. Photo/Media Release. I grant FBI Academy permission to photograph or video my child during Activities and use those images for promotional, social media, and educational purposes, unless I notify FBI Academy in writing that I withhold this permission.
  5. Acknowledgment. I have read this waiver, understand its terms, and sign it voluntarily on behalf of myself and my child. If any portion of this waiver is found unenforceable, the remaining portions remain in full effect.

I, the undersigned parent or legal guardian, voluntarily consent to my child's participation in FBI Academy programs, practices, training sessions, clinics, tryouts, games, tournaments, and related activities (the "Program"), and confirm the following:

  1. Voluntary Participation. My child's participation is voluntary, and I may withdraw my child from the Program at any time.
  2. Physical Condition. To the best of my knowledge, my child is physically fit to safely participate in soccer training and competition, except as disclosed elsewhere in this registration.
  3. Rules and Conduct. My child agrees to follow the rules and instructions of FBI Academy coaches and staff and to conduct themselves with good sportsmanship.
  4. Understanding of Risk. I acknowledge that youth soccer carries an inherent risk of injury that no amount of coaching, instruction, or equipment can fully eliminate, as further described in the Liability Waiver/Release.
  5. Communications. I consent to receive emails, texts, or calls from FBI Academy regarding my child's participation, schedule changes, and Academy news.
  6. Authority to Sign. I represent that I am the parent or legal guardian of the participating minor and have full legal authority to sign this consent on their behalf.
